SAGE’s mission:
To provide supportive care to help you along your path to healing and wellness. It is a place to transition from rehabilitation/chronic pain and illness to wellness. The model of healthcare in our country is reactive- only focusing care on issues once they present. We believe preventative care is the only way to health. Our facility is a safe place for all who are working towards better wellness.
OUR SERVICES:
Highly Trained Staff (variety of backgrounds and training- mostly professionals who trained as Physical Therapists and Physical Therapy Assistants who have retired from practice) are collaborating to support you on your journey to physical wellness*.
As a member of the Sage Wellness Community you will have access to:
our exercise facilities (gym and pool) *on an as available basis
guidance sessions from our practitioners.
In development: Specialized classes developed by physical therapists to incorporate some of the issues seen most often in clinic: BALANCE, FUNCTIONAL STRENGTH, COGNITIVE CHALLENGES, NEURAL CIRCUIT REPROGRAMMING
Our pool is maintained at a therapeutic and relaxing 90-92 deg. Sessions in the pool are private, reserved for one person at a time. Even people who are shy about getting in front of others in a bathing suit feel comfortable in our mostly private facility. Our pool has a current jet and a treadmill. Chair lift into the pool is available.
*Unfortunately, insurance does not cover preventative care and wellness, which we believe is critical for health. Although our practice focuses on care that is not covered in the insurance model, if we believe you would be best served with a comprehensive course of skilled physical therapy we will refer you to one of the clinics in our community who will be a more appropriate venue for your care.

1 year membership to Sage: $350/year
Includes:
1 Wellness exam (1 hr) for all new members, or choice of 1 Exercise Guidance session (1 hr) in gym or pool (for continuing members)

Membership rate for gym: $5/hr if self booked online, $7/hr if booked at reception (Reception discount for 10 visits, paid with cash/check booked at the same time: $55)
Membership rate for pool: $20/ ½ hr if self booked online, $22/ ½ hr if booked at reception, (Reception discount for 10 visits, paid with cash/check booked at the same time: $210)
Opportunity to schedule additional guidance sessions (1 hr) with wellness staff:
$125/hr with Aquatic Trainer or fitness instructor
$175/hr With Physical Therapist trained providers
Referral-In 1 year membership: $175/1st year (Membership for people transitioning from physical therapy or from PCP referral, referral form needed to be filled out and signed)
